The three rating Systems on the left side have been configured to accept multiple votes, try them out, you'll be able to vote as much as you want. On the right side, instead, you can vote only  once, because the script checks the voter by the IP address and , if it finds it in the database your vote will be rejected. This isn't a perfect  method to have unique votes because most of the IP addresses are dynamic and change whenever you disconnect from the internet and connect again, but at least the visitor won't be able to vote too many times.

You can add this rating system to php pages but It can be done even with .html .htm  pages as long as you allow the web server to parse PHP with such extension.For further details about this topic visit this page
The obvious reason for three rating systems below is to show you the different sets of stars. You can choose the color when you install the 5Star Rating or change them whenever you like by editing the config.php file or from the new Admin Control Panel , from where you can look at how your site pages or articles have been rated or change the setting via an easy Interface..
